Medicare Part D is a voluntary program that provides prescription drug coverage through private insurance plans. Most people enroll through a standalone Prescription Drug Plan (PDP) or through a Medicare Advantage plan with drug coverage (MAPD).
For people with diabetes, Part D covers: insulin (all types), oral diabetes medications (metformin, SGLT2 inhibitors, GLP-1 agonists, etc.), and diabetes-related testing supplies if not covered under Part B.
The Inflation Reduction Act of 2022 capped insulin cost-sharing at $35/month for Part D enrollees starting in 2023. Starting in 2025, the catastrophic coverage cap effectively limits total out-of-pocket drug spending to $2,000/year for most Medicare beneficiaries.
Note: Insulin used in a Medicare-covered insulin pump is reimbursed under Part B (as a supply necessary for the pump), not Part D.
