Insulin is a hormone produced by beta cells in the pancreas that enables cells throughout the body to absorb and use glucose for energy. In Type 1 diabetes, the immune system destroys these beta cells, making external insulin therapy essential for survival. In Type 2 diabetes, insulin resistance and progressive beta cell decline eventually necessitate insulin therapy in many patients.
Types of insulin differ by onset and duration: rapid-acting (lispro, aspart, glulisine) peaks in 1–2 hours; short-acting (regular) in 2–4 hours; intermediate (NPH) in 4–12 hours; and long-acting (glargine, detemir, degludec) over 18–24+ hours.
Medicare covers insulin under Part D (the prescription drug benefit). However, insulin used in a covered insulin pump is billed under Part B. The $35/month insulin cap under Part D, established by the Inflation Reduction Act for Medicare beneficiaries, applies regardless of which insulin is prescribed.
