A meter that measures blood ketone levels — used to detect DKA risk in insulin-using patients, especially pump users.
Blood ketone meters use electrochemical test strips (beta-hydroxybutyrate-specific) to measure blood ketone levels from a fingerstick sample. Unlike urine ketone strips that measure acetoacetate (an indirect marker), blood ketone meters measure beta-hydroxybutyrate directly — the primary ketone in DKA.
Blood ketone testing is recommended: during illness or infection, when blood glucose exceeds 250–300 mg/dL despite correction, when an insulin pump occlusion alarm fires, or any time DKA symptoms (nausea, vomiting, abdominal pain, rapid breathing) are present.
Normal blood ketones: below 0.6 mmol/L. Caution zone: 0.6–1.5 mmol/L (contact provider). Danger zone: above 1.5 mmol/L (seek emergency care). Above 3.0 mmol/L indicates DKA until proven otherwise.
