A larger dose of rapid-acting insulin delivered on demand to cover a meal or correct a high blood glucose.
A bolus is a discrete dose of insulin delivered either by injection or via insulin pump to address a specific glucose-raising event. Meal boluses are calculated based on the carbohydrate content of food and an individual's insulin-to-carb ratio. Correction boluses are calculated based on current glucose, target glucose, and an individual's insulin sensitivity factor (correction factor).
On an insulin pump, boluses can be delivered in different patterns: a standard (normal) bolus delivers all insulin immediately; an extended (square wave) bolus spreads delivery over time for high-fat or high-protein meals that raise glucose slowly; a combination (dual wave) bolus splits the dose between immediate and extended.
In closed-loop systems, the pump's algorithm may automatically deliver correction micro-boluses when the CGM reading rises, reducing the cognitive burden on the patient.
