Diabetic shoe inserts (orthotics) are removable insoles designed to redistribute plantar pressure away from high-risk areas of the foot — such as metatarsal heads, heel edges, and bony prominences. They are typically constructed from multi-layer foam, plastazote, or custom-molded materials.
Custom molded inserts are fabricated from a plaster cast or 3D scan of the patient's foot, providing maximum protection for feet with significant deformities. Prefabricated inserts use standardized shapes but can be heat-molded for moderate customization.
Medicare Part B covers three pairs of inserts per calendar year as part of the Therapeutic Shoe Benefit — typically one pair comes with the shoes and two additional pairs are the allowable insert quantity. Custom molded inserts are covered when foot deformity requires them; otherwise prefabricated inserts with heat-molding are covered.
