Control-IQ is Tandem Diabetes Care's automated insulin delivery (AID) algorithm integrated into the t:slim X2 pump. It uses CGM readings (from Dexcom G6 or G7) to automatically increase, decrease, or suspend basal insulin delivery and to deliver automated correction boluses up to once per hour when glucose is predicted to exceed 180 mg/dL.
Control-IQ runs in two modes: Standard (targets a wide TIR range) and Sleep Activity (more aggressive overnight targeting to 112.5 mg/dL). An Exercise Activity mode reduces correction aggressiveness to lower hypoglycemia risk during physical activity.
The pivotal PROLOG trial and post-market studies show Control-IQ achieves approximately 70–75% TIR in diverse populations, with significant reductions in hypoglycemia and A1C. The algorithm's transparency (it shows the user what it is doing and why) distinguishes it from more opaque systems.
