When a Medicare-enrolled DME supplier 'accepts assignment,' they agree to charge no more than the Medicare-approved amount for covered supplies. Medicare pays 80% of the approved amount; the beneficiary pays 20% (or $0 with a supplement plan). The supplier cannot bill the patient for the difference between their billed charge and the Medicare-approved rate.
Some suppliers are 'non-participating' — they can bill up to 15% above the Medicare-approved amount (the 'limiting charge'). A beneficiary who uses a non-participating supplier may owe the 20% coinsurance PLUS up to 15% above the approved amount.
All DME suppliers enrolled in Medicare as DMEPOS suppliers must accept assignment on a claim-by-claim basis or register as participating.
